Please Note: Only the academic in charge of the laboratory is authorized to, and may request transfers of UCSD research materials. If this is not the same individual as the Principal Investigator, who will be listed on the form, the academic who is in charge of the laboratory must sign the form as well.
Animal Care Program (ACP): If a live animal or animal tissue is to be sent by UCSD, make sure that you have contacted the UCSD Animal Care Program (ACP) for approval. Additional information regarding the UCSD Animal Care Program can be found by calling 858-534-6064.
Environment, Health and Safety (EH&S): If the material sent from UCSD is potentially bio-hazardous, including viruses, bacteria, fungi, recombinant DNA, transgenic plants, transgenic animals, invertebrates, insects, etc., Environment, Health and Safety (EH&S) approval may be required.
Embryonic Stem Cell Research Oversight (ESCRO): If the material to be sent by UCSD is a human embryonic stem cell or derivative, Embryonic Stem Cell Research Oversight (ESCRO) and/or Human Subjects approval may be required. Additional information regarding the Embryonic Stem Cell Research Oversight Committee can be found by calling 858-822-2647.
What Happens Next? Once the completed form is received, a Material Transfer Agreement (MTA) will be drafted and forwarded to the recipient organization for review and signature. When the Material Transfer Agreement is fully-executed, a notification is sent the UCSD Principal Investigator. The materials should not be sent prior to receiving this notification.
The review and negotiation of the Material Transfer Agreement is conducted concurrently with the approval process for Animal Subjects, Environment Health & Safety, and the Embryonic Stem Cell Research Oversight.
