Option #2
I wish to RECEIVE material:
Obtain a Material Transfer Agreement, from the providing organization, if the providing organization requires a formal agreement.
Complete the "I am RECEIVING Materials - Option #2" form:
Please Note: Only the academic in charge of the laboratory is authorized to, and may request transfers of research materials. If this is not the same individual as the Principal Investigator, who will be listed on the form, the academic who is in charge of the laboratory must sign the form as well.
Human Subjects (IRB): If a drug or human blood or tissue is to be received by UCSD, make sure that a Human Subjects Protocol has been, or is in the process of being, approved by the UCSD Human Subjects Institutional Review Board (IRB). Make sure that the providing organization identified on the Material Transfer Process form is named in the Human Subjects Protocol. If not, send a request to the IRB to amend the protocol. Additional information regarding the UCSD Human Research Protections Program/Institutional Review Board (IRB) can be found by calling 858-455-5050.
Animal Subjects (IACUC): If a live animal or animal tissue is to received by UCSD, make sure that an Animal Use Protocol or Tissue Transfer has been, or is in the process of being, approved by the UCSD Institutional Animal Care and Use Committee (IACUC). Additional information regarding Institutional Animal Care and Use Committee (IACUC) can be found by calling 858-534-6069.
Environment, Health and Safety (EH&S): If the material to be received by UCSD is potentially bio-hazardous, including viruses, bacteria, fungi, recombinant DNA, transgenic plants, transgenic animals, invertebrates, insects, etc., Environment, Health and Safety (EH&S) approval may be required. Additional information regarding the Institutional Biosafety Committee/Environment, Health and Safety can be found by calling 858-534-3660.
Embryonic Stem Cell Research Oversight (ESCRO): If the material to be received by UCSD is a human embryonic stem cell or derivative, Embryonic Stem Cell Research Oversight (ESCRO) and/or Human Subjects approval may be required. Additional information regarding the Embryonic Stem Cell Research Oversight Committee can be found by calling 858-822-2647.
What happens next? Once the completed form is received, the provider's Material Transfer Agreement (MTA) will be reviewed. If changes to the MTA are necessary, a revised copy of the MTA will be forwarded to the provider. If the changes are acceptable, the MTA is signed by both parties, and a fully-executed copy is forwarded to the UCSD principal investigator. Once the Principal Investigator receives the fully-executed MTA, the material is approved for transfer. Contact the provider if the material is not received within a reasonable time.
The review and negotiation of the Material Transfer Agreement is conducted concurrently with the approval process for Human Subjects, Animal Subjects, Environment Health and Safety, and the Embryonic Stem Cell Research Oversight.
